Refund Request Application

Allow 30 days for processing all refunds.
Refunds subject to the following:
The following form is provided for your convenience.

Refunds of registration fees must be applied for in writing to the club registrar and are;

• Full refund, upon written receipt of request prior to playing any organized team event (ie practices/tryouts/evaluations)
• Full refund, less $50, prior to participating in any practices/evaluations or playing first game. (Exhibition, tiering or season)
• Full refund, less $50, up to two weeks into season with medical reasons. (Proof of condition must be provided by doctor)
• No refund after season start.
• Refunds (if any) will be issued to original payor.
